Athletes MAY NOT participate in any of our sports programs, including Summer sports camps, until they have received approval through the Athletic Director’s Office. Athletic Clearance is a simple process involving both parents and athletes to ensure safety and understand the inherent risks that accompany athletic competition. Once an athlete receives Athletic Clearance, it is valid for the school year, unless the physical has expired. Make sure to choose multiple sports if you plan to participate in more than one. Athletes must repeat the process each year they plan on participating in our programs.
NOTE: Sports Physicals must be completed and signed by a licensed MD (Doctor of Medicine) or DO (Doctor of Osteopathic Medicine).
Physicals completed and signed by a Chiropractor (DC) will not be accepted.
Sports physicals are only valid for 1 year from the date that they are completed, so if your physical was done sometime last year you should have a new one completed to cover this school year.

Information needed to register:
A. Upload your doctor-signed physical form.
B. Fill-in your Medical Insurance provider and policy information & UPLOAD a COPY or PICTURE OF YOUR INSURANCE CARD.
C. Complete Medical History by responding to the questions.
D. Read & Accept policy agreements with electronic signatures by parent/guardian and student-athlete.
E: Wait for the email that states that you have been cleared for a particular sport.

All injuries must be cleared by a licensed physician.
If an athlete is returning from an injury that has forced him/her to miss practice and/or games, a doctors note must be provided in order for a student to return to action.
The doctors note must specifically state that a student is cleared to return to their specific sport.
